Yeah I will include the form, do I leave it blank then if there isn't any family members? And put N/A? I'm doing the new forms. I'm not giving them a reason for returning it again. I know what I'm doing so it shouldn't take long and my spouse is here visiting till the morning so it works out...

Just got ours back. :'( We were missing Additional Family information form even though there isn't any. And PA photos were missing.. they're not. They're clearly attached to the PA generic form right on top of the barcodes. Don't see how they could be missed. Sending it back tomorrow... Time...

No clue. May 10 here. Will continue waiting. It took me 3 tries to get through to them. Finally I got hold music and then waited a good 30 minutes.

Called just now, the lady said 3 weeks.. they're on May 5th applications. Not sure what answer is correct since yesterday's answer was May 9th and they said it should be coming this week. So... Just keep waiting? :/

Is there anyone that has run into this in the past and they still accepted the "outdated" forms?

Yeah.. I hope it didn't affect our application. We weren't aware of the update until after it was sent. Hoping for the best!

May 9th, received May 10, text alert came in later that day.

The new one! But it was the form before they updated it in April. By then everything was done. I'm trying to think positive I'm just kicking myself for not triple checking everything. I'm sure it will be fine. They've gotta have a grace period but alas, it is the government so anything is game!

We had seperate envelopes for each section. Sponsor, Applicant and then relationship proof. Then in a large folder which was then put in a FedEx package. There is an old post on here that someone shared how they put their package together. Mine was pretty similar. Lots of binder clips and paper...
