Dear Alvi Bhai, Its simple: 1) Go to Pakistan Embassy in your city 2) Take Your Iqama (Original & Copy), Passport (Copy), 4 Photographs (2 needed but take 4 to be on safe side), NICOP (Original & Copy) & I would suggest to keep a copy of PER E-mail with you even it is not required but still...
