Dear Alvi Bhai,
Its simple:
1) Go to Pakistan Embassy in your city
2) Take Your Iqama (Original & Copy), Passport (Copy), 4 Photographs (2 needed but take 4 to be on safe side), NICOP (Original & Copy) & I would suggest to keep a copy of PER E-mail with you even it is not required but still you know our SYSTEM
3) Just ask anyone from where to get the form for Police Certificate.
4) Pay the fee to them, they will fill your form, paste your pictures and attach your documents all by them self and give it to you for stamping and will also tell you which window to get it stamped from.
5) Go to the window, the guy will take your thumb impression 2 times and you have to pay on that window as well, I don't remember how much but all this embassy procedure will be done within 30 ~ 40 SAR I guess.
6) Finally they will give you the completed form with their stamp.
7) Go to MOFA and get it attested (Fee = 30 SAR)

Not sure, I believe there must be some branch in Jeddah as well.
8) After getting your form stamped from MOFA, go to the police station, Take 2 pics (1 needed), Iqama (Original & Copy) & Passport (Copy) with you. You have to go to (Idara' Al Janaiyah) Criminal Investigation Deptt. The chief will sign your form and then they will take your finger prints and give you the receipt with the date of receiving 2 weeks later.
