In your medical email, you should have an attachment called IMM1017.pdf, that's the medical instruction. If not, forward your medical email to OttPilot-immigration@cic.gc.ca and tell them you did not receive the attached document IMM1017.pdf.

There is no other form to fill in ? Like the eye color and height form (Updated Information Sheet) So it will be only: 1. PPR email 2. Passport 3. 2 Photos ?
