Nah, it's not mandatory. You can even use your previous name and change it later on. But no it's not a requirement.

Is it okay if we do it? They recently requsted a few documents but she has issues creating an account and when I create one for her, it doesn't seem to link. So is it okay if we use the same account or if I upload the files in behalf of her (even though they re-requested files from her)?

Can someone tell me how to address the embassy when they request more documents from us? I mean what do I put in the envelope? Is it just my spouse's address and VO's office or do I need to put the UCI and Application numbers as well?
