Yes, include hours. Also attach previous employer letter..You need to mention the NOC for current job also in the form. This also helps to show your personal history of last 10 years. I agree. Documents should not be stapled or clipped. Should be left unbound but in order of checklist. Although stapling, using clips won't affect your application. Point is why to use these when CIC already clearly mentioned in the instructions not to use.

Certificates and other transcripts are not required to be notarized. Only photocopies are required to be sent, as mentioned in checklist/instructions. Sending notarized or un-notarized does not impact your application.
