I think the name and dob at the back is helpful for them to identify the owner in case the pics are misplaced.. otherwise it wont matter. Information on forms is what matters.. even if thrs a typo, they verify and correct it on their own..they did for me last year (cap reached sk no luck). so...

Yes, you need both. You need to apply for PCC in Indian consulate in UK. Check their website for procedure. Local police certificate will not be valid. Good luck!

Salary slips need not be notarized. Having them notarized wont however affect in any way. Make sure experience letter contains the duties as required for your NOC. Copy of experience letter is asked by cic, not original. Most company generate salary slips electronically so no probs. Good luck!

In the spreadsheet, go to Data in the menu and then Filter Views. It gives various options to view the spreadsheet. This will help you the way you want to view the data. Different people will have different preferences so these filters give a few options. Thanks to Admins.

From previous year, I don't think such missed information created any issues or was required to be sent again. If somebody missed any personal history or some other minor information, CIC asked to send an email with the required information. But missed signature etc was not accepted.
