I think the reference number of 2011 file will not work, you must have applied under FSW 2013, if you have not applied at all under FSW 2013, you may still have chance if your NOC is there in the 24 eligible Occupations. I don't think that there is any harm in mentioning the local Canada address...

The transcript request form is for University ....it will go to university ...you need to send your transcripts and copies of marksheets and degree certificate to WES, it should be duly attested by Registrar or Controller of Examinations.
