These are all very helpful. What about the date of birth certificates? Are some people putting the wrong DOB?

I thought it might be helpful for everyone who has been rejected to post the reasons here. That way everything is in one easy to access spot. Might help other people get their applications right the first time! Who wants to start?

I just realised I forgot to include a copy of my 2009 work visa. Can I send it now? They haven't ask for it yet I just want to preempt any rejection.
