Do I need to send my visiting card (business card) or supervisors visiting card with the application pack?

Confused about photo order in the application pack. Any idea where to put it or with which form to attach? Some suggestions need to use paper clip to bind the forms or leave it unbind. Still could not decided. Seems both are okay.
