i applied online with very basic documents , dont exactly know much about paper base documentation, but would like to advice you to help your nerves calm. i assumed that medical report is something else and IOM receipt is different , if this is the case i dont think you should be worried about...

in my views : if your means to extend date in visa application.... you cant. (and i dont think you need to extend it, if they took long to make a decision they might ask you then you show your relaxation from your supervisor

in my views residential address is the one where you feel comfortable to receive post / mails/ courier etc. even though if you not present on that address but you know that some one is there to receive on your behalf. could be your parents house or others/ and also its must match with passport...
