1- for high BP the dr will ask for more test as urea.creatinin,ecg , chest xr & will be more carefull about the clinical exam 2= if the drugs for high chol. is given upon blood results it is no problem, but if the history showed a more serious medical condition this will depend on that condition .

if it is more than 10 years u dont need proof , u just need to mention it in pesonal history in schedule 1

as instructed in cic website you should keep a photocopy of ur appl to be sent to chc if your file recommended for further assessment if u have any changes do it in new form & write coverletter
