Re: Affidavit Format for DoB certificate Hi PMM, I am having my secondary school certificates which has the fathers name and the true date of birth. I will submit them along with the affidavits. Will it be ok then?

for Q#1, you can add an additional sheet and write the names in native language.

My employer told me that I am still legally eligible to work while my WP extension is under process. So, I am assuming it is legal to apply for PR as well since my status is documented. But, I will check with the call center.
