hi, no, there is no need of extra paper, the form itself will do unless u have a very lengthy travel history. the form has 2 pages: in the 1st page, u can fill ur own travel history (a. You), and ur spouse's TH (b. Your Spouse or common partner). And in the 2nd page, u can fill ur son's TH...

YEH, before 18th July 2011, "Supplementry Information - Your Travels (IMM 5562)" form wasn't mandatory. But for pre 26june 2010 applicants, it has to be sent with the full doc. in case of some VOs ONLY. ::: rockstar :::
