I have attached the RPRF fees with the full doc. If you have not submitted they would ask with the medicals. The AOR is for the full application submission.
I got AOR on 27th Jul. I sent my application on 14th Jul, though it reached CHC on 15th Jul, in AOR it is mentioned as 16th Jul. The AOR is dated 21st Jul and sent though registered post.

You should have one form as main applicant and another form for your spouse as the main applicant. dont confuse 'Principal' applicant with 'main applicant'. The 'applicant' or 'main applicant' is the person who is filling that form may be either 'principal applicant' or 'spouse'.

I am not sure which form you are talking about, the PP form (http://passport.gov.in/cpv/PPForm.pdf) has 12 items and you have to fill up all the required info.
Can somebody let me know pls is it necessary for all the applicant to enter Canada within the 1 yr deadline or just for the principal applicant. thanks for your response.
Moreover it's not a day's or a week's job. I am sure it would take atleast couple of months to change/add it in your passport. before you have to notify through a govt gazette, publish in a newspaper for certain days, get a new birth certificate etc.
As Aanbaan told I am not sure about the principal applicant. My wife and daughters don't have the surname in their passport and I have sent the application to NS and got response and also sent full application last week, no issues until now.
Well, once your medical is done, they will request your passport for the visa stamping, I dont think you can travel anywhere outside your country without your passport, leave alone visa.
