I am not sure if any form was filled because my consultant was handling the entire profile until I got my ITA. After getting ITA I have taken control of the entire application process. But i think kryt0n may be right. Just saying 'no' where it asks for representative should do it.

I think one bank account is sufficient. The reason they have mentioned all bank accounts is that they want the applicant to not hide any debts or liabilities. I would say, disclose the bank account which has sufficiently large funds.

Why do you have to go through the entire process again? Can't you just ask for another report to be mailed out? They already have your results with them.

If they are not comfortable writing exact working hours eg. Mon-Fri 9-5, then ask them to write number of hours per week.
