I sent out my application on Saturday October 23rd and it was received today in Vegreville. We didn't do the medical form because my husband had one done a little while ago for his work permit for here. I included a copy of the work permit so I hope that will suffice.

we had a lawyer who helped us up until just recently... unfortunately we could not come up with the legal fees as fast as need be so we kinda had to make a choice. Even though she wont be at the hearing, if we have any questions we are able to contact her.
