Yes. Upfront Medicals are absolutely mandatory. Without the medicals, you won't be able to submit your PR application. Medical exams will not take more than 2-3 hours of your time. Try getting the appointment from the panel physician in your area at the earliest. All the best!

Yes, you can renew your passport at this time. If you have not got the ITA, update the EE profile with the new passport details as soon as you get it. If you have already received the ITA, you can submit the new passport details in the PR application. Just give a Cover letter to your passport...

Ideally, if the Panel physician is using the eMedical system then he will send the IMM 1017B Upfront Medical Report form directly to CIC (they normally do it in 3-5 working days) and will give you the information printout sheet which you are supposed to upload in your PR application.

Its mandatory to get Medicals done before submitting the PR application in case of Express Entry applications (that is why its known as 'Upfront Medicals'). Moreover, I do not think you would be able to submit the application (technically system will not allow) without uploading the Medicals...

For me the medicals Passed on 15th October and the Background check is in status 'Not Started' since 19th October. Any idea, if there is any other stage between the 'Medicals Passed' and 'Background Check in Progress'?

Yes, the application can update anytime during the day. However I have read in some of the threads on the forum that most of the updates happen on Tuesdays and Fridays. Co-incidentally, I had my application updated on Fridays so far :)
