Yes it is medical request. This case is not for express entry. You will receive an email requesting you to undergo medical.

I made a specific Enquiry last Friday about my file transfer.. Then yesterday manila visa office emailed me that my file has been transferred to them.

Nope., I think manila visa office doesn't send email regarding file transfer. They will just email applicants regarding MR and RPRF.

Regarding your first question. I think you really need to provide birth certificates. To prove that you are really related to your supporter. Coz I have a friend who submitted her application before to MPNP but after a year her application was refused coz she forgot to submit birth certificate...
