Its not an issue. She can apply for a visa extension and it is usually granted. (it's a bit weird because at that point its obvious she is not a "visitor" and wont be leaving Canada. But they tend to approve visa extensions for PR applicants regardless.)

Inland is fundamentally different from outland. You cant just switch one for the other. For example, Inland applicants cant appeal. Outland applicants can get their SIN with their COPR, Inland have to wait for their PR card. Inland applicants can getan OWP at stage 2, Outland can't. Etc.

Yeah its kind of messed up. If she is visa-exempt then its probably fine to tell them you plan to apply inland. So dont worry about it.
