Hi Yes it is the correct form. I faced the same situation. I submitted a wrong form and had to withdraw the application. And the place of your visit, I put down my work name and address. It should be fine. Cheers

Hi According to the website it says it take 7 business days and last updated on 14th April 2017. So they are working on applications received on April 14th not Jan 29th.

Applied on April 1st with no upfront medical. Provided with only one picture but later on through CSE provided 12 pictures. Did not submit PCC but will do next week through CSE again. Hoping for the best.
