only refugees are covered for medicals.. your overseas dependents will have to pay for their own immigration medical exam.. can you please share your timeline?
it does not necessarily take a year for your family to get here. It depends on the visa office that is processing their application. I have seen people here in the forum have their family here in Canada in 2-4 months after the principal applicant get the CoPR
I guess if your dependents in Canada, then their names would have been included in your ecopr. But since they’re outland, then there they’ll get a separate copr from the visa office when their processing is completed.
