Hi kcire092681, I also had that issue... apparently there's no form to fill-up when you pay online. Only a receipt will be available. I just attached a letter to CIC stating that no form appeared and I included all the required informations stated in the guide that are supposed to be provided...

Conagratulations! Are you now able to register on line with CIC and Check your application status? ... how long do you suppose is the waiting time before the next step (Med Request ??? )?

I see... So that was why I was strongly adviced by the earlier applicants to indicate my sponsor's email address for mail correspondence in my MPNP online application. ;D
