Another member here Arman has the same issue. He posted here a few days ago. Just take a look at the previous pages. You can send an enquiry to the Visa Office or ASAP contact your MP. You can explain your situation to your MP and he/she can contact the visa office on your behalf.

What date was the decision given? Is it just recently? Did the embassy contacted you by email or mail before when they asked for the RPRF?

Yes, you need to fill out this form regardless if those people are in your application or not. I believe this form is a part of the background checks. So you need to mention your spouse, mother, father, siblings and kids (if applicable)

Thanks SooMazi, I am relieved then I used the right communication channel. I will get the new set of notes next week but I doubt they will have anything new. It seems like our file has been abandoned since September... :(
