I'm a bit unsure about this form. Do I (the person being sponsored) ned to fill in all the information including the part about siblings?

It's my wife that is sponsoring me and only me, not any other family members.

Yes, you need to fill out this form regardless if those people are in your application or not.
I believe this form is a part of the background checks. So you need to mention your spouse, mother, father, siblings and kids (if applicable)
