hmmm :( . I have sent the signed IMM5669 through web form. Got a reply for that as an acknowledgment. Looks like this method is working for other who did the same mistake. I hope my application will not be returned. BTW, have you sent the signed form again when you realize the mistake.

Yes. They received it on December 11th.

The webform is asking either application number or UC ID. i do not have those things with me in my case. Tried calling - but as soon as I press 0 to speak to an agent, it disconnects. :(
