You need to be present in US to maintain H1b status. I doubt your company will be okay with you staying out of country for extended periods of time, while on H1b.

In certain cases, they extend the medicals at officer's discretion. In my case, I did my medicals in May 2018 and AOR Dec 2018. They extended the medicals in June 2019.
