Express Entry - While self-employed, I moved countries (Work History, claiming points for foreign work). How to include in the work history?

Since you worked for the same company, I would list it as one entry. If you have to indicate the country (filled those forms almost 2 years ago, so memory not very clear), use Czech Republic since this is where your company is registered. You will get the opportunity (and you will have) to...

Try to figure out why they could not verify these two work experiences and upload updated reference letters or any other documents that would help the immigration officer verify the information. If your situation is complex, get in touch with an immigration consultant.

How to get a provincial nomination after applying in Express Entry

The first tip is to go back to your express entry profile and make sure that you have selected all the provinces (except Quebec) in the list of provinces where you would consider moving. That way, your profile will be included in the searches of all the PNP that are aligned with Express Entry.

As @ag2020 wrote, you should simply send via webform the paper that the medical center gave you to confirm that you came by to do the exam. It will have all the necessary references the IRCC needs to locate the actual results that will have been transferred directly by the medical center to IRCC.

If your appointment is on May 12th, then you should be able to upload that day, via Webform the document issued by the medical center, confirming that you did the medical. So, you will be well within the 30 days time frame.
