I notice sa IMM: 5669 (12-2012) magkakasama yung state/ province and district but in IMM: 5669 (2017) form magkakahiwalay na sila. So, nalilito ako kung saan ilalagay... Naiba kasi yung form and yung way off filling it up. Yung 2017 form kasi yung gamit namin kasi yun yung most recent form...

Me too. They requested Schedule A last May 30. I am checking it now for some errors and sending it on Monday... Hopefully it will be transferred after. Manila VO is my VO if ever it got transferred.
