depending on citizenship of your parents, medicals are assessed in respective regional med office. For e.g. LVO can be London, but medicals assessment in new delhi. are you registered on MyCIC? If yes, around what date the background check was "in progress"?

you provide your birth certificate even though your father name is on it.also provide marriage certificate of your parents. on that it will show both your mom/dad's name. this will connect the dots. on top of that you are providing transcripts and passport copy.
