Your additional family information form requires all your names to be written in english and your native language, if you are intentionally leaving out the info then its at your own risk. To give yourself a peace of mind, just write down their names in your native language atleast with a pen.

Instructions fo filling up the form are pretty clear, they need your information as per your passport/ travel document. If your passport doesnt have any family name then you should write Not Applicable or NONE or NA.

Btw, How do I update this information on the left that appears under my username? <----------- This information?

Thanks a lot bro. As for the card, I ordered it through amazon (faster). So it took you 5 weeks to get the PCC letter or just to process it?

My application is already on the way ... As I told you, I followed their instructions to the best of my understanding and wrote NA in her family name as it is blank in her passport.

As per the instructions given on the top of the form and guide, yes you can attach an additional paper and enter the information in the format requested. You will need to write you name, name of the form and the question you are answering on top of this additional paper.

I did it the other way round. typed NA or Not Applicable in her family name. The reason behind doing so was to mention the information as per her passport.

Anyone have any idea about how to get PCC from US? Should I get a state police clearance or the one from Federal Bureau?

It took them 1-2 days to change from "evaluation report being prepared for delivery" to "Completed". Once the status says completed they will give you the tracking number if it is a courier. My report was completed on May 14th and sent on May 15th and I received it on May 19th. I got it...

My B.Tech Degree has my family name written in abbreviated form whereas my MS degree has the full name. So should I answer Q2 on IMM0008 as yes and enter the abbreviated last name or should I answer no, since its basically the name but only in abbreviated form. Waiting for kind replies.
