Hi, I came to this country in Sep 2021 on a work permit, applied for PR in Dec 2022, received my PR in May 2023. I will be eligible for my citizenship in few months. Before applying for my PR, I did go to my home country twice (Dec 2021 and Jun 2022) for 1 month each on vacation. Since this was the period before the PR, my attorney told me to include these dates as working from outside to avoid any issues even though I was on vacation and not working. This was done to correctly capture 1 year experience required for CEC (vacation time outside was not counted for 1 year CEC). And since this was a vacation, my residence was still mentioned as Canadian address (2021 Sep - current).

How do I address this in my citizenship application? Something like "employment Sep 2021 - present, home address Sep 2021 - present? Neither has my employer changed nor my home address. Thanks!

It doesn't matter whether you worked from home or didn't, you are still employed during your vacations (or during weekends!).
Ignore these trips altogether in both the address and work history questions, and only list them in the physical presence calculator section.
