Selected UCI and Family name option.
Enter Place of Birth same as it is in the passport. For me, there's a space between city and state (City, State) while there's no space in my spouse's passport. I ignored space at first attempt and it didn't found any record. Then tried again with space, accepted.

I’m waiting also, submitted mine on the 25th of Oct I saw some people got it next day
