Canada passport application help - part 5 and 7

a couple of questions in passport application
1. Part 5- Identity - It says write name as appears in document . I am using Ontario Driver's license . The name format is last name, given name.
Should I use this format or given name-last name
2. Part 7 - Address
Address in the last 2 years - I have moved to my current address this year.
Do I need to write my current address again ? for "to date" should i just leave blank ?

What are you using the driver's license for? I think need to follow how the name is written in the citizenship certificate that you need to submit with the application.

OK, I checked the application, and it seems yes, you need to provide your name as it appears in the document you are providing as a proof of identity.
