RAMQ without CSQ

Hi,
I know that you can have your RAMQ card even if you don't have a CSQ, but I've been hearing lately that RAMQ are starting to insist on having CSQ, and they request an authorization form (IMM 5475) to be filled for them. any experience with that issue?

Hi, Did you solve RAMQ issue? Do you have a card now? how did it go? please inform me. I am also in same situation. I don't have CSQ and need RAMQ card as non-CSQ PR card holder.
