I did an upfront medical and was provided with an INFORMATION SHEET and Health Case Details, but not the IMM 1017B Upfront Medical Report form. What is that? How do I get it?

I don't think upfront medical is the norm for family sponsorship. Are you applying outland or inland. Is your spouse/sponsor living in Canada or outland with you?
