Additional Family Information [IMM 5406] QUESTION

Hi, Please Help me
My brother is sponsoring my mom, im 20 and im a dependant child so i have to fill out the IMM 5406 and i dont know what to put under APPLICANT, SPOUSE and CHILDREN since im single and im not the applicant.
The form says I must put "NOT APPLICABLE" in sections that do not apply to me. Is it safe to sassume that the form is correct since it does not have my name under it other than my signature?
DO just write Not applicabe in every box and leave out the select sections?

ALso it correct to assume my dad is a dependant/spouse he is my brother's father so is he an applicant to or just the spouse?
keep in mid my mom is the main applicant.

PLEASE HELP DEADLINE IS COMING

If you look at section for section under Additional Family Information (IMM 5406)

You can see that the first set of name entered, isn't JUST for applicant. It's for "yourself". So you need to put your parents name under "your mother" and "your father" (whom one of them is the principle applicant of this PGP sponsorship application.)

Who needs to fill out this application form?
This form must be completed by:

  • You, as the principal applicant,
  • Your spouse or common-law partner (whether accompanying you to Canada or not), and
  • Your dependent children aged 18 or over (whether accompanying you to Canada or not).
SECTION A
Write the personal details for:
  • yourself,
    • If you are married and you were physically present at the marriage, indicate “married – physically present” in the marital status box
    • If you are married and you were not physically present at the marriage, indicate “married – not physically present” in the marital status box
  • your spouse or common-law partner, (if applicable)
    • If you are married and your spouse was physically present at the marriage, indicate “married – physically present” in the marital status box
    • If you are married and your spouse was not physically present at the marriage, indicate “married – not physically present” in the marital status box
  • your mother, and
  • your father.

Hi All, I am currently filling out the Application Form IMM 5406 Additional Family Information. My Partner (Sponsor) and I are applying for Conjugal Sponsorship and just a brief background, I was previously Married which had broken down. What would I put on the SECTION A for Spouse and Common Law. Should I put the name of my ex husband? I tried to put the name of my current partner but there is no option for her marital status as SINGLE/ Conjugal Partner. I am a bit confused. Anyone please enlighten me. Thanks in advance.

When I applied in paper based application Nov 2018 at that time my mother was in Canada on visitor visa for less than 6 months. So I have mentioned her address as India address in family information form because she is visitor visa . Now she wants to extend her stay more 6 months. So do I need to inform CIC that my mother's address is Canada because she is living there more than 6 months ? Or it will be s India address in family information form?

While the spouse fills up the form, does she enter the details of her parents or the parents of the applicants? Sorry it might sound stupid but it is better to be safe than to be sorry :)

Hi there, i am about to submit imm5406 form. In the instructions it says ‘bearing original signature’.. it means i need to print it out and sign the form?
Please help me out..

I have been nominated from sinp under international skilled worker occupation in demand.

Now i am applying for pnp paper based application.
As i am principal applicant and my dependents are my spouse and my 6 year old daughter. so i filled schedule A form for myself and my spouse.

Q1 So who will sign on the last page of my spouse schedule A form.
Q2 on checklist it has been written that schedule a background /declaration
Completed dated and signed by everyone in the following list
-The principal applicant
-Spouse
-child over 18 yrs of age

So whether schedule a form need to signed by both(principal applicant and spouse)
Q3 similar is question for additinal family information
In check list it is stated that
Completed dated and signed by everyone in the following list
-The principal applicant
-Spouse
-child over 18 yrs of age

I have filled 2 form seperately for my self and my spouse

Whether these need to be signed by everyone means both me and my spouse on each form

I don't really know the answer to your question. Since you are applying under PNP I am not sure if the guide for these forms are different from family sponsorship. (e.g. under family sponsorship dependent's age is under 22 and yours refer to under 18)

For the form IMM5406, it is mentioned that "Complete ALL names in English and in your native language.". However There is only single cell to enter each information. Where should we write the names in native language?

I put it in the same box for the name. I typed it in English and after everything is verified and printed, I hand wrote name in native language.

If in doubt, read the instructions. If not in doubt - still, read the instructions.

The checklist is pretty clear. Point 8 says the principal applicant must fill out the form. In your case, only the principal applicant.

Note, IMM0008 - you do not include your children on this form as they have citizenship. That is the form that only applies to those applying for PR status.

yes, the applicant (in ur case ur wife) has to fill the IMM 5406.
include all her relatives, regardless of whether they'll come to canada or not.

You already got replies from others. :) Just follow the guide and PA needs to fill IMM5406.

Hi,

I received a mail from IRCC stating
Dear (my full name)
This is in reference to your application for permanent residence in Canada. In order for us to continue the review of your application, we require the following documents:
 (My Spouse UCI Number): My Spouse name
 Additional Family Info IMM5406: A separate and newly completed Additional Family Information form (IMM5406) bearing an original signature.

I filled out my wife's details in the form as per the request letter.
When I tried to upload the filled out IMM5406 form in my account, it shows that the IMM5406 can be uploaded only under my section.

What should i do? Do i fill out my details or my wife's details in the IMM5406 form?

Any Suggestions?
