Sorry to hear about it, Looks like an issue with SOP. Order GCMS notes before reapplying.

Yes, but if you haven't received approval there is a high chance is a rejection.

In my case, the first rejection - revised at the same time, second - first letter then a day later rejection.

But you never know...
