Questions about forms - Inland Application

Hi,

We have additional questions about the application forms:

Schedule A / Background Declaration
  1. The full name of my wife's father is known, but no other information is. If a father's name is specified, then it also requires the date of birth, however, it doesn't accept asterisks, only a real date. What to do in this case as the form won't validate with that field not filled in? We cannot say father is unknown because his name is on the birth certificate
Use of Representative
  1. Use of representative. Since the application is not submitted yet, should the representative section be filled with "Not Applicable" for the application office and the, type of application, and Client Identification number or just leave blank? We am just being extra careful with not leaving things blank, as we have heard that CIC want anything not applicable to be explicitly stated as N/A or Not Applicable
Open Work Permit
  1. Coming to Canada section. Provide the document number of your most recent Visitor Record, Study, or Work Permit. Do we need to specify my wife's visitor visa number here or just leave blank or say "Not Applicable"?
  2. Background information section. Question 2c we answered "Yes" because my wife applied for the visitor's visa prior to applying for this Open Work Permit, to enter Canada. Is this correct or should we specify "No" here? With the "Yes" answer, we explained that my wife applied for a visitor's visa, which she was granted on such and such date, and the number of the visa.
Relationship Evaluation

  1. We are currently living together, and we have specified that for Question 3. However, Question 4 is asking "If you are not living together, have you visited each other during your relationship?". Should this be left blank without specifying either "Yes" or "No" and putting "Not Applicable" for the text, as well as the itemized periods of visits? Once a "yes" or "no" is clicked there, you cannot unclick it, it has to be either yes or no. I guess for this to be blank, a new form will need to be started.

Father's name: provide a date (any date) and explain in a letter of explanation.

Representative form: if you are not appointing a representative, do not submit the form. Write "N/A" on the document checklist.

Open work permit: if there is no visitor record, study permit, or work permit, write N/A.

Open work permit: I'm not sure what this question actually asks, sorry. I filled one of these out recently but don't have the form in front of me.

Relationship evaluation: Put N/A. Start a new form. It should actually grey that section out once you select that you are living together, but N/A is all you need.

My wife did apply for visitors visa before, and this is why we put "Yes" on that form and provided details regarding when she applied, when granted the visitor's visa, and what is the visa number.

For the Relationship Evaluation, I re-created the form and did not specify a "Yes" or "No" for the visiting section. It did not grey it out when I checked that we are living together, not visiting. I just put "N/A" in the description for visiting and "Not Applicable" for all the visiting dates and locations.

Separate letter of explanation for each form.

You can represent her, no issue.

You answered correctly.

Correct to put N/A for the visits because you live together.
