No measure to judge the strength of ties or level of satisfaction. I think it's discretion of the officer.
There is definitely some factor that is just based on the discretion of the officer. Certain assumptions are made and it can be unfair. If others have violated terms of their visas in the past from a certain country, future applicants from that country can suffer.

It’s absolutely at the discretion of the VO. They have been appointed to represent Canada and its interests. Perception and discretion will play a significant part I. Their decision making process. If you are unhappy with the result, you can apply for a judicial review. If the decision the VO is valid in law, they will find against you (assuming they even accept your application to the court). If it isn’t, they will overturn the VO decision and send it back for reassessment.
Otherwise, apply again with a stronger application.

Really sorry about your refusal. I have been through the same situation in 2017. To prove your family ties you need to obtain the Family registration certificates from NADRA which prove your family ties in Pakistan. However you can also mention about your spouses , children and other dependent relatives in your covering letter. You also have to describe about the importance of this trip and give a brief day to day summary about your plans and interest in Canada. Although it will be very supportive if you hold a valid US visa and travelled to any other European countries in the past. Most of the people got rejected because they don’t submit Canada family information form and travel history form, you should fill these form and also make your own travel history summary as well. If you have any ongoing appointments or scheduled meetings in Pakistan do add the documentary evidence of that as well. So these are the few basic things which most of the applicants forgot to attached and face refusals. I wish you all the best for your future applications.

Everything was there less covering letter. NADRA REGN too was there. Children married and abroad. I had visited UK twice in last 5 yrs. Only problem is that I and my wife are considered by the VO would not come back. How to convince him otherwise is not understood. I'm grateful you took time to explain. I wish VO has discretion to ask clarification or explanation before giving his final verdict.

How to apply for judicial review. If you could mention that link it will be a great service to many of us.
Judicia reivew will cost you thousands of dollars with no guarantee of approval. You'll need to start by hiring a lawyer in Canada to represent you which will be very expensive. Plan to spend $3-5K in Canadian dollars.

Best option is to reapply.

As Scylla mentioned, you’ll need a Canadian lawyer to apply for JR. and it won’t be cheap. Having only applied for a TRV once, and not really providing all the details needed (by your own account), it’s probably a safe bet the courts will reject your application for review. That said, here’s just about everything you need to know about a judicial review.
