Affidavit format for non accompanying spouse

Hi Friends,
Does anyone is having affidavit format for non accompanying spouse? Me and my spouse are not living together and we will be starting our divorce process soon. I want to attach the affidavit stating that i dont want to include her in my application & wont be sponsor her in future. Please share the affidavit format.
