I need to attach an additional page for SECTION - 3 of IMM 540 (Additional family information form) for my mom, do I need to get this additional sheet to be signed from my mom , or signing the actual form in itself is sufficient?

I believe she will have to sign that also.
I just got my parents signatures on the 5406, once i receive the courier back from them, I’ll be writing thw names in native on the same form.

But if you use additional sheet, I believe she will have to sign that too.
 
Hey guys kind of urgent. I am unable to validate form Schedule A and reason its saying null value in signature. But How can we do signature without printing the form?
When I submitted the form, my parents have no education history and nothing to put in first name. I asked IRCC and submitted invalidated form with explanation. See below for more

For you, since it is just complaining about signature. I would put name in there and then validate then print and sign.

Just type in the name in the signature field, validate and print
And get the handwritten signatures next to the typed name in the signature box.

I am sponsoring my Mom and I don't have a birth certificate so planning to get an affidavit. I am staying in Toronto, so could anyone who is getting the affidavit for the birth certificate provide the address/contact# from where I can get it done.

If the birth certificate does not have parents name, do I just write a letter and provide copies of another document like my old passport where parents name was listed (or OCI etc) to prove the relationship ? OR submit an AFFIDAVIT ?

I won’t recommend scanning the documents.
Once the form is validated, go to print and select “print in pdf” option.
That way you would be able to save it to your computer in a “read only” version.
Send that to your parents through email.
I sent all the pages so that the quality/ink color won’t change from the rest of the papers.

But if you really want to send the last page, from that saved pdf version, split-extract the last page using some online tool (just google split pdf), and send the last pages by email.

But my recommendation is to send it all together, that was advises by someone in this thread, before that i also wanted to send just last pages.

Could anyone please tell me , what should i put in family size ( 1 or 2 ) ?

For IMM 540, some of the family members doesnt have email-address, do I have to write N/A for this or leave it empty?

If I write N/A for email address fields, then i ll have like 10 N/As of that one page form.
I left it empty. :)
