Thanks for the inputs but I'm still confused regarding point 6

6 . Declaration from Non-Accompanying Parent for Minors Immigrating to Canada (IMM 5604)

Are there any applicants under the age of 18 included in this application, who will receive an immigrant Visa?

Yes

What do we need to fill with above point? In this case as my new born is under 18, do I need to choose Yes?

Prinicipal Applicant:

If other parent of principal applicant is not listed as co-signer on IMM 1344, provide the following

Completed IMM 5604 ,witnessed by notary public

If my wife is not a co-sponsor, do I need to fill IMM 5604 and signed by notary?
You do not need to fill this form since your wife is already a PR. The form is for a non accompanying parent who is either not a permanent resident or not going to be living in Canada with the child. The form is Not Applicable to you

I have questions on filling some of the forms:

IMM 0008:

2 - Category under which you are applying?

Do I need to fill dependent child ? Here my new born is the principal applicant

3 - How many family members, including you, are in this application ?

I'm the sponsor and my wife is co-signer. As I'm sponsoring my new born, do I need to put 3 (Myself,Spouse & new born) or just 2 (myself and new born)

Have a question if IRCC requested IMM5604 form and was already sign and notarized by the non accompanying parent ,it looks like my daughter application I forgot to submit with her documents,both her mother and grandmother had court matter "long story" with my daughter ,I got both to sign and notarized separate forms ,it's looks like I just submitted the moms form in the beginning now they asking for the grandmother sign form I looked tru my files and there it was ,does this form has an expiration.

Can I submit March 24th 2018 IMM5604 form ,already notarized and signed ?

Why march 24th? Why not immediately? When was the second form signed? You can send it and include a letter that it was already signed same time the Mother signed hers. You only missed out the form while arranging the others. I don't see why they won't accept it

Oh. That's like a year already. You can send it to them and do what I earlier suggested while you try and get another signed form just incase it gets rejected. Then immediately you get that you can quicjlq upload it aswel even though you haven't gotten a response from them about the previous one

I am a July 2018 sponsor for my wife who came to Canada in June 2018. She has a daughter in Vietnam who is 11 Years old. we filed all the necessary documents including an IMM 5604. However, since the requirements are that the signature must be witnessed by a notary public or similar person, we had a problem getting a witness to sign an English document. So we had the IMM 5604 translated into Vietnamese and then a Deputy Minister of the Judiciary of the city my wife's ex resides in, signed and stamped the document (in Vietnamese). We had the whole document translated back into English in Canada by an authorized translator and included it with our original application package.
Our application seemed to go well until February 2019. At that point everything stalled and no matter what we did, we could not get the file moved forward. Every phone call I made, I was re-assured that everything was okay and that they (CIC) were not looking for any additional documents.
When the 12 months since application came and went without any contact from CIC, I requested my file details under the Freedom of Information Act (30 calendar days to get an answer).
I also made an appointment with my local MP to see if she could help. She wrote a letter on my behalf and told me that it would take 10 days to get an answer.
Coincidentally, tomorrow is the deadline for both actions and my wife got a request letter today, asking her to send an IMM 5604 form, which of course they already have.
I am lost with what to do next. Has anyone experienced something similar?
