Cannot find a check-box on the application for citizenship (minors)

Hi,

This is rather a silly, trivial matter but I am trying to fill out the application form for my child and unable to locate the yes-no check-box that should be there for question #10.

I cannot find it next to the acknowledgement statement that goes, "I am the parent or guardian of this child. I have read and understand the prohibitions under the Citizenship Act."

The reason why I am asking is because such a box exists on the adult version of the form.

Could somebody kindly confirm whether the check-box really exists? I am using Adobe Acrobat Reader DC Version 2019.010.20098.

Hi,

There isn't any check box for that statement in #10 rather the same is mentioned in #12 and the parent is supposed to sign. So you signing #12 is an act of acknowledging it.

Thanks, GraceCan. For #12, could you kindly confirm whether the city and date fields, respectively, have to be filled in, if the child is under 14? I know the child does not need to sign if under 14.

Yes you need to fill them. You are signing and these fields are not optional.

I filled the child section too (place and date) but its not required for kids who are younger that 14yrs. You can fill it or skip it - it wouldn't make a difference.
