I checked ECAS and it says "we sent you medical instructions on February 19, 2019. To avoid delays, please provide us the information requested in the letter as soon as possible. Please consider delays in mail delivery before contacting us."

Neither me or my wife (PA) have received any email. Can someone confirm if there is a delay in emails? Also, from where can I book the medical test.
Did you link your application with GCKey? Because you can find your messages with the appropriate information + forms to print in your messages if you click on the PR application and go down to the bottom of the page.

As far as scheduling the medical exam goes, you contact the office most convenient to you directly and tell them you need to schedule a medical exam for immigration purposes to Canada (some panel physicians do exams for multiple countries so I think it's best to specify). You can find a list on the official CIC website by searching "panel physicians."

I've done eye

I have another question which is better St luke or IOM.. cause I heard some negative feedback about iom
IOM is much better. They have a better holding area compared to st. Lukes. They wont require you to have vaccines either. Aside from the price, you wont spend the whole day for the medical exam unlike in St. lukes

I cannot link the application to GCKey as I have a representative. However, my representative told me she cannot link the application because she has not received any email asking to do that. I will double check with my representative now.

Having a representative was a bad idea, and if she is not helpful, I will just remove her and manage the application myself.

Had a chat with a lady and saying that they have my new forms, but she told me that she doesn't know if they will pick it up to start processing as soon as they see the forms or I if I'm back in the cue!

Did anyone who received MR today or yesterday had their application moved to local VO before MR?

Just curious because my application was sent to Cairo VO on the Feb 7 and still no MR.

I am curious about that too -- we got MR yesterday and ours hasn't gone to the local VO yet (as far as we know anyway). I know that @svilli's application has moved to the local VO already and her PA didn't get MR in this latest batch, so maybe there's something to that (or not... who can tell! :p).

Mine was sent to Warsaw VO on Feb 12, no MR too.
Now, I think, first I'll get AOR2 and then MR..

I think every VO has it's own rules on whether they want Sydney to send MR before they start the BGC or not. Which explains why We did not receive MR yet. Even though I already got AOR2 on ECAS and BGC has started. I noticed from the 2017 spreadsheet that Cairo VO often sends MR and PPR on the same day! o_O

Oh that's interesting actually. I don't know anything about Cairo's processing times, but maybe it's mostly VOs that are on the slow side, because they don't want the medical results to expire while they're still working? Just a theory!

Most straight forward Outland Spousal Sponsorship follow these process steps (the order of the steps may vary)
  1. Application received at IRCC
  2. IRCC start processing application
  3. Receive Acknowledgement of Receipt (AOR 1)
  4. Link Application to Online Account
  5. Sponsor Approval and File transfer to VO
  6. VO starts processing PR application (AOR 2)
  7. Receive Request for Medical Exam
  8. Medical Exam @ IRCC Panel Physician Clinic
  9. Medical Exam Result
  10. Passport Request (PPR)
  11. Pick-up Permanent Residency Visa (PUM)
1 to 4 goes in a sequence
5 to 9 can vary ( as per my numerous conversation with IRCC )
10-11 goes in a sequence ( the most exciting part)

Spousal sponsorship process completion time varies depending on the Visa Office and on a case basis

your theory is realistic;)
There were a lot of cases before my PR (in 2015) when people passed second medical check.
For me it took 6 months between AOR2 and Medical Request (they did nothing during that time :(). Background check was started after MR. Six weeks after medical passed I got passport request. In total PR process was 10 months (now a real processing time for the same program is 17-18 months)

I think, that visa offices processed Family applicants separately, because on IRCC web sites duration for these categories differs and we will get soon our BR, MR, RPRF, COPR and other "welcome to Canada" abbreviations :D

Did your PR app also go through Warsaw?
