I'm still not quite clear on what you said. My understanding is that CIC sent you an email requesting IMM 5768 to be sent again, as it is missing some information. You corrected it and sent it back. But, after you proof read it, you found that:
1) CIC made only one correction
2) You found another error
If it is "1", then I would not worry about it as you have provided all the information CIC requested. It's up to them to use the information appropriately. However, if you think your application will be negatively impacted by this apparent oversight by CIC, you might want to contact them to have it corrected as per the latest IMM 5768 you provided.
If it is "2", then fill out the form again and send it via email or IRCC webform.
