Thanks - as my photos are now dated more than 3 months after they were taken (on the back) and the form is dated 15 May will they be out of date now - will they reject it if I just resend? There was nothing missing from the application the item they sent it back (my physical presence print out) was in there - so frustrating. They must have missed it! The whole form is dated 15 May so it is now more than 3 months old - does it matter?

When you call IRCC helpline, you should ask these questions to them. My 2 cents would be to take the photo's again (but the help files state the pics should be within the last 6 months so you are actually safe). See "Specifications Pt 1" here:

The form if it is the latest version should be fine but you will need to add the dates from the time you sent to the current date for your presence and employment so I guess you will need a new filled out form. I know - terrible - but it needs to be done. Best get it over with completely and without errors.
For the physical presence form, send the old one and a new one and indicate that the old one was part of the original package in a note to be attached explaining why you are "resending" the package and mention that you would request that the original date of the application be considered for the processing, if possible as the package was complete when you sent it but the physical presence form was inadvertently missed. I am not sure they will consider this as it will be "your word" against "the agents" but if you are brief, respectful and to-the-point, there is a chance they will consider the original date.
All said, it is the processing officers discretion no matter what. You will just have to bite the bullet here and move on. Have a good one, Little Un.
