In which case we need affidavit from translator?

My translations all have official letter head, footer, stamp and sign of translator (who is a licensed one), stamp of judicatory and foreign ministry.

I talked to my translator and he said we don't provide such thing because everything is already verified by judicatory and foreign ministry. However, If you know the format, send it to me and I'll take a look.

Questions:

1. Do I need still need to provide affidavit from the translator?
2. If so, what is the format? Is translator stamp enough or it should have notary and other stamps as well?

The rule is, if the translator is a certified one and his stamp on the translated document indicates that, in that case no need for an affidavit. If he isn't certified, then an affidavit should be provided.

The official stamps that you got, in my opinion, should suffice.

Yes, he is certified, otherwise he can not get translations verified by Ministry of Justice and Ministry of Foreign Affairs.
