I beg to disagree with your 3rd assertion. The R10 check done at CIO is to confirm you submitted every required document based o the information you filled in your application. If you have passed this stage(which I believe you have since u confirmed your application has been transferred to LVO) then YOU CAN NOT be rejected based on PCC. If your visa officer needs clarification, he has to send you an ADR. My little cents.1. You must raise CSE to LVO, if you are sure that your case is in LVO.
2. You can not check what was submitted.
3. Yes I think 90% cases are refused due to PCC, if missing.
4. I think you must raise CSE or email to LVO and try give them an explanation. This is your only chance.
