Mention all the rejections. Better to tell yourself about rejections than them to discover.

First you waiting last three days to Ask plus the question is clear , they will know anyway and will reject the file and ban you 3 years and will doubt your credibility in any western visa application later Forever if you think it is smart to hide . Lies won’t bring you far and this question could not be misunderstood.
Tell them everything clearly and they like honest people and accept it whatever the reason but if you lie and they discover nothing will help even reapply could be refused based on previous mispresention , USA give life long ban for mispresentation.

You should be really fortunate that the officer did not catch hold you for a possible misrepresentation of previous visa refusals. Generally they shoot people straight with a misrepresentation email for not declaring visa refusals. You should use this chance and declare both the visa refusals.

No it is not but good to explain in LOE as they keep all previous forms is maybe they not mention but giving you chance tell all with reasons it is not late as long they want you fill again , they do for both cases now to say truth to consider or to be sure you put NO again and misrepresent to refuse and have evidence twice against you.

This is not a major issue. Many people get their visa's rejected and with US a lot of rejections take place. To issue or not to issue you/your wife was decided made by the VO at the time of interview based on the documents provided and answers to his/her questions. Getting denied/rejected does not affect your next application. But if you do not disclose it then it may affect your application. I would suggest in an LOE you to mention the country, type of visa your wife applied for and the dates when she was refused.
